to your specific situation. The information listed below represents
documentation needed for traditional, “full-documentation”
loans.*
- Last 30 days of computer-generated pay stubs.
- Last 2 years W-2 forms. If starting a new year
and previous year’s W-2 is not presently available, provide
year-end pay stub for previous year along with additional W-2
from 2 years ago.
- If self-employed or using commissioned income
to qualify, also provide last 2 years Federal Income Tax Returns.
PLEASE PROVIDE ALL PAGES, SCHEDULES AND ATTACHMENTS, INCLUDING
1099 FORMS.
- If self-employed, provide a recent year-to-date
Profit and Loss Statement for the business.
- If self-employed, please provide last 2 years
Federal Income Tax Returns for the business. PLEASE PROVIDE ALL
PAGES, SCHEDULES AND ATTACHMENTS, INCLUDING SCHEDULE K-1’S,
AS APPLICABLE.
- If any of the following income sources will
be used to qualify for the mortgage loan, please consult your
loan officer for specific documentation requirements: Alimony,
child support, social security benefits, retirement/pension income,
etc.
- Last 2 months bank statements for checking,
savings, money market accounts, as applicable. PLEASE PROVIDE
ALL PAGES OF ORIGINAL STATEMENTS. If providing printouts from
your banking institution, the printouts must show a consecutive
60 day activity history and be STAMPED AND SIGNED BY A BANK REPRESENTATIVE.
Copies printed from the internet are generally not acceptable.
- Copy of most recent retirement, pension, investment
account statement, as applicable.
- If using gift funds as a source of funds for
closing, please consult you loan officer before transferring any
funds. Gift funds must be accurately documented and paper trailed
from the gift donor to the borrower.
- If purchasing a property, provide fully-executed
Sales Contract/Purchase Agreement for subject property.
- If refinancing, please provide a copy of your
most recent mortgage statement(s), a copy of your current homeowner’s/flood
insurance policy declarations pages (as applicable). If your current
mortgage carries a prepayment penalty, please provide any information
you may have relating to this issue.
- Please consult your loan officer regarding
the collection of any upfront fees such as the appraisal fee and
application fee.
- Fidelity Bank will need to make a photocopy
of each borrower’s Driver’s License.
